WHAT CONDITIONS DO YOU TREAT?

  • Running, lifting, and sports injuries

  • Gait/balance dysfunction

DO YOU ACCEPT INSURANCE?

We are a direct care clinic and do not accept insurance. HSA, FSA, cash, check, credit, and debit are accepted for payment of services. If desired, we are able to provide you with documentation for you to submit to your insurance company for them to reimburse you for your costs. Your insurance coverage determines the extent to which they will reimburse. Medicare does not reimburse for cash-pay visits.

DO I NEED A REFERRAL?

Dr. Ericka is a direct access physical therapist. This means she is able to evaluate and treat you without a prescription for physical therapy. If you are being referred to physical therapy by another provider, please have your provider send your PT prescription to the fax number (855) 975-3070 or bring your prescription for PT with you on your evaluation day. 

WHAT SHOULD I WEAR TO MY SESSION?

Comfortable clothes that are easy to move in! Preferably active wear and sneakers.
